Sitagliptin is chemically described as (R)-4-oxo-4-[3-(trifluoromethyl)-5,6-dihydro[1,2,4]triazolo[4,3-a]pyrazin-7(8H)-yl]-1-(2,4,5-trifluorophenyl)butan-2-amine. Its molecular formula C16H15F6N5O and molecular weight is 407.31 g/mol. It is approved by the United States in 2006. It was the 74^th most commonly prescribed medication with more than 9 million prescriptions. It was developed by Merck & Co. It was approved by Food and Drug Administration (FDA) in 2006.

Sitagliptin is used as a medicine to treat type 2 diabetes. When the body does not make Insulin can cause high blood sugar levels (Insulin is hormone, which controls sugar levels in the body). It is used to treat high blood sugar levels caused by type 2 diabetes.

Sitagliptin is an 'anti-diabetic medication' as Januvia. It is available as a generic medication. Adult dosage 100mg taken once per day. It is not prescribed for children and in senior citizens, it depends upon the kidney function.It is used with perpeutal diet in addition to routine exercise and other medications as well to control high blood sugar. It significantly reduces fasting plasma glucose and 2-h postprandial glucose. It also improves insulin secretion and beta-cell function.

Mechanism:-

Dipeptidyl peptidase 4 (DPP-4) is an enzyme which breaks down the GLP-1 and GIP, Gastrointestinal hormones released after intake of meal. By preventing break down it helps the secretion of insulin and suppresses the glucagon by alpha cells present in pancreas. It steer blood glucose levels to normal. Sitagliptin is used to lower HbA1c level by 0.7% to 0.8%.

Common side effects includes dizziness, stomach pains, headache and swelling of the legs.
